My Information and Advance Care Plan (Easier Read Version)
This advance care plan by Nottinghamshire Healthcare NHS Foundation Trust is a written plan about what the person with learning disabilities wants, needs and would prefer to happen when needing care in life threatening and life limiting illness. It is to be filled in by both the person with learning disabilities and their support worker or carer.
